Anterior Open Bite

An anterior open bite is a complex condition in which the upper and lower incisor teeth do not fully touch when the jaw is closed. It can range from an almost indistinguishable gap to a noticeable and unsightly space in the front of the mouth caused by protrusion of the front teeth. In severe cases, none of the teeth might touch besides the molars.

What causes it?

This condition is usually caused by a mix of genetics and environmental conditions like extended thumb-sucking, inability to breathe through the nose, and/or improper resting position of the tongue. A degree of anterior open bite may be part of the process of a child’s adult teeth growing in, but if it doesn’t resolve itself, the position of the teeth will need professional treatment to align properly.

Besides being unsightly, an anterior open bite can affect chewing ability and even the stability of the teeth. For less pronounced cases, treatment can include behavior modification and “passive” therapies like devices that help place the tongue correctly.

What Treatment Options are Available?

If the open bite is more pronounced, there are several treatment options. In some cases, orthognathic surgery may be indicated. Otherwise, a mechanical force needs to be applied to the posterior teeth over a consistent period of time. Options include external headgear or miniscrew implants, which are hinge-like devices that arch across the roof of the mouth in conjunction with braces.

Due to advances in technology, less intrusive treatment may be a possibility for some people. Emerging evidence shows that Invisalign used with rubber bands may be as good or better than braces at correcting the problem. To prevent any recurrence, it is important to change any contributing behaviors and continue maintenance as directed by the orthodontist, even after initial results have been reached.
